State guide

Arizona

Arizona separates inspections into Level I, II, and III. Level I handles basic VIN-to-document matching for title-only, bonded-title, and some out-of-state or out-of-country cases, while Level III is the restored-salvage inspection done at ADOT Enforcement and Compliance Division locations.

When a VIN verification is required
Arizona uses Level I inspections for title-only requests, bonded titles, and some out-of-state or out-of-country vehicles without proof of title or registration; Level II or III is required when VIN issues or branded foreign titles create unusual circumstances; Level III is required for restored salvage, recovered stolen, or collision vehicles, and ADOT materials also list homemade trailers and special constructed vehicles among inspection-triggering situations.
Who can do it
Level I may be performed at MVD or authorized third-party offices, or by designated law enforcement officers; Level II is performed by a certified ADOT Officer; Level III can only be conducted by a peace officer at ADOT ECD inspection locations.
Official form
Level I Vehicle Inspection (48-4503); Level II/III Inspection Request Transmittal (96-0409) for authorized third parties requesting higher-level inspections.
Fee
Level I: no fee at an MVD office (third-party providers may charge a fee); Level II: $20; Level III: $50; AZ VIN: $5.

What to do next

  1. Use the ordinary out-of-state title page first; many normal out-of-state transfers do not appear to need a special inspection.
  2. If your case is bonded title, title-only, or missing proof from another state or country, expect a Level I inspection before processing.
  3. For restored salvage, book a Level III appointment through AZ MVD Now and bring receipts for repaired or replaced component parts.
